DOT physical in Florida

The exam itself is federal — same standards, same certified examiners in every state. What's state-specific is the paperwork: in Florida, your medical certificate goes to the Florida Department of Highway Safety and Motor Vehicles (FLHSMV), and missing that step downgrades your CDL even with a valid card in your wallet.

Filing your medical card with FLHSMV

Florida CDL holders self-certify and file the medical examiner’s certificate with FLHSMV — online through the department’s medical certification page, by mail, or at a driver license service center. FLHSMV sends renewal reminders, but the deadline that counts is the date on the card itself.

You'll also self-certify your driving category (interstate or intrastate, excepted or non-excepted). Most for-hire freight driving is non-excepted interstate — the category that requires a current medical certificate on file. If you're unsure, your carrier's safety department will know which box you belong in.

Intrastate exemption exists — but read it narrowly

Florida law exempts certain intrastate-only operations from the federal medical certificate requirement. Most CDL jobs still count as interstate commerce (freight moving across state lines makes the trip interstate even if you never leave Florida), so confirm your actual category with FLHSMV or your carrier before relying on the exemption.

Heat is a real exam factor

Blood pressure readings run higher when you arrive rushed and dehydrated — a practical Florida concern. Arrive early and hydrated; the certification thresholds don’t bend for the weather.

The exam is the same everywhere

Any examiner on the FMCSA National Registry can certify you, in Florida or any other state — the card is valid nationwide for up to 24 months (shorter if a condition like high blood pressure needs monitoring). What the examiner checks doesn't change at the state line:

Frequently asked

Do intrastate-only Florida drivers need a DOT medical card?
Some intrastate operations are exempt under Florida law, but the exemption is narrower than it sounds — hauling goods that started or will end their journey out of state is interstate commerce. Confirm your category with FLHSMV before skipping the physical.
Where does a Florida CDL holder file the medical card?
With FLHSMV — online, by mail, or at a driver license service center. An expired certification leads to CDL downgrade, so file the renewal before the card’s date.
