What to expect at a DOT physical
The whole exam usually takes 30–45 minutes. Here's each step, in order, so nothing catches you off guard.

Paperwork and health history
You fill out a health history form listing conditions, surgeries, and medications. Be honest — the examiner cross-checks it during the exam.

Vision test
You need at least 20/40 in each eye with or without correction, plus a 70° field of vision in each eye. Bring your glasses or contacts if you use them.

Hearing test
You must detect a forced whisper at 5 feet in at least one ear, or pass an audiometry test.

Blood pressure and pulse
High blood pressure is the most common reason for a shortened card. If yours runs high, rest a moment and ask for a re-check.

Urinalysis
A simple urine sample screens for protein, blood, and sugar — signs of conditions like diabetes or kidney issues. It is not a drug test.

Physical examination
The examiner checks your eyes, ears, mouth, throat, heart, lungs, abdomen, spine, neurological function, and extremities.
How to prepare
- Bring a photo ID and a written list of your medications and prescribing doctors.
- If you have high blood pressure, take your medication as usual and avoid caffeine beforehand.
- If you use a CPAP for sleep apnea, bring your compliance/usage report.
- If you have diabetes, bring recent lab results (such as your A1C) and any required forms.
- Wear your glasses or contacts and bring your hearing aids if you use them.
