DOT physical in New York

The exam itself is federal — same standards, same certified examiners in every state. What's state-specific is the paperwork: in New York, your medical certificate goes to the New York State Department of Motor Vehicles (DMV), and missing that step downgrades your CDL even with a valid card in your wallet.

Filing your medical card with DMV

New York CDL holders self-certify and submit the medical examiner’s certificate to the NY DMV medical certification unit — commonly by email or mail, with the barcoded certificate form. The DMV must show a current certification on your record; the paper card alone doesn’t protect you from a downgrade.

You'll also self-certify your driving category (interstate or intrastate, excepted or non-excepted). Most for-hire freight driving is non-excepted interstate — the category that requires a current medical certificate on file. If you're unsure, your carrier's safety department will know which box you belong in.

Article 19-A for bus drivers

New York layers its own Article 19-A program on top of the DOT physical for bus drivers: carrier-administered exams and periodic reviews. If you drive a bus in NY, expect both the federal medical card and the 19-A file with your employer.

Keep the barcode legible

The DMV matches submissions to your record from the certificate itself — send a clean, complete copy and check your license record afterward to confirm the new expiration date registered.

The exam is the same everywhere
