Getting your CDL in Missouri
In Missouri, apply through the state licensing agency (MO DMV / DOT). The core steps are the same across states:
- Get a valid MO driver's license and provide required identity documents.
- Pass the DOT physical with a certified medical examiner and carry your medical card.
- Pass the written knowledge tests (general knowledge plus any endorsements you need).
- Hold your commercial learner's permit (CLP) for the required waiting period.
- Complete Entry-Level Driver Training (ELDT) with a registered provider, if required.
- Pass the skills test — pre-trip inspection, basic controls, and road test.

What is different in Missouri
Licensing in Missouri runs through the Missouri Department of Revenue. The part that catches people out is not the test — it is the medical certificate, because the way it reaches the state changed in 2025.
Missouri runs driver licensing through the Department of Revenue. DOR began accepting electronically transmitted medical examiner certificate information from the National Registry on 21 April 2025, and with it removed the requirement to submit paper copies. Self-certification is still yours to do: every CDL and commercial learner’s permit applicant and holder tells DOR which of the four types of commercial operation they drive.
The online medical update option is gone permanently
As of close of business on 10 April 2025 Missouri suspended MyDMV online renewal and duplicate requests for CDL holders, and permanently removed the option to submit a medical certificate update request online. If you remember doing this on MyDMV, that route no longer exists — the examiner’s electronic submission replaced it.
